We look forward to working with you and/or your son or daughter as they compete for Concordia University Chicago! It is our hope that he/she will be successful in every aspect of their collegiate experience.
Mission:
The purpose of the Athletic Training staff at Concordia University Chicago is to provide the best medical care possible for our Cougar student-athletes in which to compete in intercollegiate athletics. The primary goal of our program is to prevent athletic injuries from occurring. However, with the knowledge that injuries are inevitable, our medical staff is equipped to perform immediate care, evaluate and treat athletic injuries and provide post-injury rehabilitation for our student-athletes, and also to advise them regarding available treatment options. Our goal always is to provide the medical resources necessary to allow student-athletes to return to competition as soon as it is safe to do so.
Pre-Participation Medical Forms:
All of the student-athletes who intend to participate in intercollegiate athletics at Concordia University Chicago must have completed a physical examination performed by a physician, a thorough medical history, have read and signed all risk waivers, and demonstrate proof of primary insurance that covers athletic injuries. All of this information must be on file with Athletic Training prior to participation. All student-athletes may login to the electronic medical records system and update/complete all required medical information on or after June 1. The deadline for completion is August 1.
Proof of Sickle Cell Status:
*FRESHMAN AND TRANSFER STUDENTS ONLY*Â This is a new requirement, but no NEW student athlete can begin practice until proof of sickle cell status has been submitted.

The final decision on participation is the responsibility of Concordia University Chicago's Team Physician. The Team Physician also makes the decision on when an athlete may return to competition after an injury/illness. The Dean of Students has the final authority to determine the eligibility of a student who wishes to participate in intercollegiate athletics.
Facilities:
The Athletic Department of Concordia University Chicago has two athletic training locations on campus. The first is located in Geiseman Memorial Gymnasium and is used primarily for indoor sports. The other is located in the Cougar Athletic Training and Services (CATS) building and is used primarily for outdoor sports.
Much of the injury rehabilitation and treatment that is carried out at the University also is performed in the CATS training room. Both training rooms are equipped with the necessary functional rehabilitation equipment to provide Cougar athletes with the quality health care that will allow them to get back in action as quickly and safely as possible. Note: Injuries incurred outside of athletic participation will be referred to off-campus medical facilities at the expense of the student-athlete.
Insurance:
All Concordia University Chicago student-athletes must provide evidence of current primary medical insurance that covers athletic injuries and provides a minimum coverage of $90,000. This is a National Collegiate Athletic Association (NCAA) prerequisite for athletic participation.Regrettably, injuries sometimes do occur, and we attempt to provide our student-athletes with the very best possible care. Medical bills are incurred when the athlete is treated whether it is locally, during a road trip or by a medical vendor in his/her home area.
Concordia University Chicago's membership in the NCAA does not permit any college or university to provide coverage or pay the bills incurred for expenses related to illnesses or conditions which are not sustained as the direct result of an injury in our intercollegiate sports program.
The athletic accident insurance provided by Concordia University Chicago for your son or daughter is for injuries while participating in the play of official practice and competition in intercollegiate athletics while under the supervision of a coach. This coverage is supplemental to parents or the student's major medical coverage and provides a benefit only after the $100 deductible is met, per each injury.
